The Donegal Memorial Monument Committee (DMMC) seeks to publicly recognize leaders in the Donegal community through inclusion on the Donegal Memorial Monument. These individuals have made substantial contributions that have made a lasting, positive impact within the Donegal School District and/or the community.
To make your nomination, please print out, complete and then submit this nomination form by mailing it to DAC 929 East Main Street #135 Mount Joy PA 17552 by June 30, 2023.
To demonstrate the nominee’s qualifications, please describe on the nomination form in one or two paragraphs how the nominee has contributed to the Donegal School District and community. You will need to specify the nominee’s characteristics and traits of integrity, virtue, achievement, leadership, community engagement and service to the community that have made a lasting impact on the students and/or members of the Donegal community. Your response will be limited to the space provided on the form.
Eligibility Criteria
The DDMC will consider all nominated individuals who possess characteristics and traits of integrity, virtue, achievement, leadership, community engagement and service to the community that have made a lasting impact on the students and/or members of the Donegal community.
Nomination Procedures
Submitted nominations will be reviewed by the DMMC during the month of September. After nominations are reviewed by the DMMC, selected nominees will be presented to the Donegal School Board for review and confirmation. The nominating sponsor will be notified if their nominee has been selected for inclusion on the Donegal Memorial Monument.
Nominations remain active for five (5) years. To remain under consideration by the DMMC, nominations that have not been acted upon after (5) years must be resubmitted.
